Reminder: New CMS-1500 form required as of April 1, 2014

The CMS-1500 claim form has been updated for ICD-10. Form Version 02/12 will replace the CMS-1500 claim form, 08/05, effective for claims received on/after April 1, 2014. Available for use now, the form has been updated to adequately support the use of the ICD-10 diagnosis code set that will be mandated beginning October 1, 2014.
