Register for CURES

In order to retain your authority to prescribe hydrocodone combination products, you must register for the California Department of Justice (DOJ) prescription drug monitoring program known as PDMP/CURES by January 1, 2016.

If you have a DEA number, you must register to use PDMP/CURES. PDMP/CURES tracks prescription information of schedule II - schedule IV controlled substances through an online database to help fight prescription drug abuse and trafficking. Some of the information required to be submitted to PDMP/CURES by prescribers and pharmacists includes: patient name, birthdate, prescriber name, prescription information and name of the dispensing pharmacy. Below are instructions on how to register for the database.

1. Pre-register online
Complete the registration application at https://pmp.doj.ca.gov/pmpreg. 

2. Print and notarize the registration confirmation
Upon completion of the online registration, immediately print your registration confirmation.  The confirmation page is the user application form which must be signed, notarized and submitted to DOJ with copies of supporting documents (see below).

Supporting documents:

a. Copy of optometry license;
b. Copy of Drug Enforcement Administration Registration (prescribers only); and
c. Copy of government-issued identification.

3. Mail completed application packages to DOJ

4. E-mail Verification
Applicants will receive an e-mail requesting verification of their e-mail address.  This notification is automatically generated once the online registration has been submitted to DOJ.  The email verification must be completed within 72 hours of its sent date.
